Pharmacy Discount Program:
How our 340B Specialty Pharmacy program works
*We contract with Coordinated Care Network Pharmacy (CCN) out of Pittsburgh, PA

You will have your own team with a Care Representative and Pharmacist.

Your Care Representative will call you every month to review your medication needs and set up delivery.

Your Pharmacist is available to answer any questions about your medications and discuss ways to best manage your treatment.

They will help you to use financial programs from drug manufacturers and other programs.

They will also help you and your prescriber with insurance authorizations.

You will choose how you would like your medication packaged (medication bottles, pillboxes, or individual easy-to-open packets).

You will decide where and when to receive your FREE delivery.

You can choose from a Courier Service, USPS, UPS, or FedEx.

You can make delivery changes anytime to fits your needs.

You will receive monthly refill reminders and a personalized adherence plan.

Mental Health
Therapy

Services are tailored specifically to the needs of the Lesbian, Bi-Sexual, Gay, Transgendered, Queer/questioning (LBGTQ+), Black & Indigenous People of Color (BIPCO), Differently Abled, Latinx, and New American (refugee and immigrant) communities. In addition, this sub-contracted relationship provides linguistically appropriate services by bi-lingual staff members. Services allow us to:

Conduct a mental health (bio-psycho-social-sexual) assessment with each client referred to Journey to a Trauma-Informed Life.
Provide trauma-informed, culturally sensitive outpatient mental health counseling with children, adolescents, adults, couples, and families of the clients accessing services at the PA Thrive Partnership.
Provide trauma focused evidence-based treatment (including but not limited to Eye Movement Desensitization and Reprocessing (EMDR), Cognitive Processing Therapy (CPT), Trauma Focused Counseling Behavioral Therapy (TF-CBT)) for clients diagnosed with traumatic stress disorders.
Provide a trauma-informed, culturally sensitive eclectic approach to working with individuals experiencing concerns related to their unique intersections of diversity (including LBGTQ+, BIPOC, Differently Abled, Latinx & New American community members.
Offer Yoga, Art and Reiki therapy in a group setting
Provide telehealth/tele-counseling services utilizing a HIPAA compliant platform.
